Information listed in the checklist must be submitted either via email at [email protected] OR by mail to Premier Association Management, 1795 E. Hwy 50, Suite A, Clermont, FL 34711. The form, along with associated attachments, can also be delivered to a member of the ARB or HOA Board.

- Plot map of Lot showing residence and setbacks with improvements/modifications noted on the map
- Supporting documents – material data sheet, contractor license/insurance information
- Owner contact information – VERY IMPORTANT!
